Support Worker CLS/Home Care/Night Care Assistant

Job purpose
To work with service users with learning disabilities to enable them to fulfil their person centred plans, promoting positive outcomes and maximise independence.
Job Context
The Community Living Service is a 24 hour/365 residential service in ordinary houses to adults who have profound and multiple learning disabilities. Within such a service there is a range of individual needs which will include challenging behaviour, communication difficulties and some people we support will have acquired brain injury. The consequences of this means that some service users will need substantial help with their personal care (including assistance with toileting and bathing) as well as supervision having regard to the understanding of risk.
The Community Living Service is regulated by the Domiciliary Care Agencies (Wales) Regulations 2004 and their accompanying National Minimum Standards. Individual workers within the service are required to register as Social Care Workers with the Care Council for Wales.
The service is currently based in ordinary houses in which 1 or more service users who are the tenants of the properties and are cared for by a small group of staff.
There is a minimum number of staff required within the house having regard to the needs and activities of the service users.
Principle Accountabilities
Direct Work with individuals (and families)
To form, develop and maintain professional relationships with people who have disability, through direct supervision and shared activity, in such a way that the person's needs are met.
* Assisting and supporting individuals in day to day personal/practical care tasks according to individual needs, which may include assisting with personal hygiene, dressing, cooking, household tasks etc.
* Assisting and supporting individuals to access and accompany them in identified community based activities e.g. leisure or day and employment services, in line with individual care plans. In particular, to assist individuals in the use of general public services.
* To implement the service user's individual care plan agreed by Social Services and Health Professionals and assist in the development, implementation and review of the service delivery plans and health action plans.
* To ensure standards of personal care and all aspects of safety are maintained at all times, in accordance with the Department's Health and Safety Policy. Assisting Senior Support Workers to manage risk through implementation
